Click the appropriate link to open the application form. Type in the required data and print the completed form using Acrobat's Print tool. ILAS 201 and 301 applications also require a 1 - 2 page personal statement which can be created in Word or WordPerfect. FAX all required copies to me at (815) 753-4802.

Clinical Applications - a separate application is required for each clinical:
Fall semester: ILAS 201 (1)
Spring semester: ILAS 301 (2)

Be sure to include your personal statement. Original + 4 copies required.

Intensive Student Teaching Application - includes both fall and spring semester experiences:
Fall semester: ILAS 401 (1) and ILAS 300 (2)
Spring semester: CHEM 497 (12):

Be sure to include all 3 parts of the application. Original + 3 copies required.
